Tattooing may involve health risks such as: Pain, bleeding, swelling, infection, scarring of the tattooed area, and nerve damage.

I have been informed and understand that the tattoo procedure should be considered permanent: that it can only be removed surgically and that any effective removal may

leave scarring or disfigurement.

Tattoo pigments/ inks are more than 99.9% organic with less than .1% non-metallic preservatives.

I understand that the final outcome of the tattoo may differ slightly in color, actual size , and design from the design chosen.

I agree to follow aftercare instructions as explained by my Tattoo Artist.

I agree to hold harmless Black Rhino Tattoo, or any employees of Black Rhino Tattoo, responsible for any complications that may arise with the tattoo from the procedure.

He sido informado(a), entiendo y además acepto que, como con cualquier procedimiento invasivo, el tatuaje puede implicar riesgos para la salud tales como: dolor, sangrado, inflamación, infección, cicatrización en el área tatuada y daño a los nervios.

He sido informado(a) y entiendo que el procedimiento de tatuaje debe considerarse permanente; que solo puede eliminarse mediante procedimientos quirúrgicos y que cualquier eliminación efectiva puede dejar cicatrices o desfiguración.

Los pigmentos/tintas para tatuajes son más del 99.9% orgánicos, con menos del 0.1% de conservadores no metálicos.

Entiendo que el resultado final del tatuaje puede diferir ligeramente en color, tamaño real y diseño del diseño seleccionado.

Acepto seguir las instrucciones de cuidado posterior (aftercare) tal como me las explique mi tatuador(a).

Acepto eximir de toda responsabilidad a Black Rhino Tattoo y a cualquier empleado de Black Rhino Tattoo por cualquier complicación que pudiera surgir como resultado del procedimiento de tatuaje.
